Medication Refill & Ordering Guidelines

DEADLINE: DATES ARE TBD, ONE WEDNESDAY A MONTH AT 3PM

If you email/text after 3pm, and NEED meds, you will have to pay the late shipping fee of $15.00.

NEXT ORDERING DATE:

APPROXIMATELY 1 MONTH LATER, GIVE OR TAKE. ALWAYS LOOK AT THE NEXT EXPECTED DATE IN THE EMAIL REMINDER.

MEDICATION PICKUP/DELIVERABILITY:

ALL MEDS WILL BE MAILED AT COST DURING THIS PANDEMIC. MAKE SURE YOUR ADDRESS IS UP TO DATE. WHEN IN DOUBT, SEND A QUICK NEW EMAIL.

ALTERNATIVE RESOURCES FOR AFFORDABLE RX TO BE PURCHASED IN THE PHARMACIES: 

MEDICATION AVAILABILITY

Medication refill/dispensing are as per availability from wholesaler. If medication(s) not available on date of ordering, then you will be notified, and decision of where to send a prescription to your pharmacy will be made.

MEDICATION MONITORING LABS

I've tried to be very accommodating, but there must be adherence to the medical plan for me to continue to prescribe certain medications. Alternatives to this are election to termination of your membership. There will be slight laxity with regards to the pandemic, but labs that are consistently not being rechecked in review of your chart -- no further medications will be prescribed or dispensed.

BILLING NOTE

Patient accounts MUST BE PAID IN FULL and up to date, in order to be eligible for non-controlled medication dispensing from the clinic. Other terms as noted below.

PLEASE READ ALL TERMS THE BELOW:

  • Review ALL your meds that you want to try to get refilled and dispensed through our clinic.
  • Notify me of exact med and dose (mg/mcg) if capsules/tablets/inhalers. Make list if necessary.
  • Quantity: So long as you are a current member who has NOT notified our clinic of termination of membership, you can be eligible, likely, for 90 day supply of meds (pending review by physician).
    If you HAVE given/received termination notice, you are eligible for 30 days total refill as by most medical practice standards and medical/legal risk. No exceptions, as noted in your membership termination confirmation.
  • Pending the above in # 3, let me know if you would prefer 30 or 90 day supply.
    Nasal sprays: 1 bottle = 30 days; 3 bottles = 90 days.
  • Reminder: Controlled substances, inclusive now of Gabapentin (Ohio Pharmacy Law changed), are NOT eligible for dispensing through our clinic
  • Inhalers are considered hazardous shipping, and they do not arrive overnight like other meds; they come via ground. Thereby there is a delay, and you will have to pick up in person.


Please make sure you have reviewed the above policies before requesting medication ordering or dispensing.

Pharmacy refill requests can also be done using this form, simply state PHARMACY at end of the medication details.

If different pharmacy is needed, please include those details: Name, Address or zip code.
